We collect information you provide voluntarily, such as your name, email address, phone number, and credit card details (for payment processing only) during lead generation and patient registration.

We collect information automatically as you navigate the site or through our analytics providers. We may store usage information such as the type of device you use to access our Website, including IP address, device ID, the pages you visit or request, links clicked, referring sites, user interactions, and your search terms. We also derive your location from your IP address.

Appointment Policy

To ensure that you get an appointment for the date and time you desire, we suggest booking all appointments a few weeks in advance. Our website displays the most up-to-date availability. Please note that we require a credit card, and require $100.00 deposit fee, to reserve your appointment.

We value your time and want to ensure you receive the full benefit of your scheduled appointment. To maximize your treatment time, we kindly ask you to arrive on time.

Please note that appointments exceeding 15 minutes late may need to be rescheduled to accommodate other clients and ensure a smooth schedule. We recommend allowing ample travel time to avoid any unexpected delays.

Deposit Policy

Some of our services may require a deposit to secure your appointment. This deposit will be applied towards the total cost of the service. Deposits are non-refundable in the event of cancellation, unless otherwise specified in our cancellation policy. 

 

Cancellation/No-Show Policy:

Ensuring Timely Appointments: At Superior Self Aesthetics and Wellness, we have a 48-hour cancellation/no-show policy. This helps us ensure timely appointments for all our guests and respect our providers’ time.

Fees for Late Cancellations: Any appointment that is not cancelled or results in a no-show within 48 hours will incur a fee automatically charged to the credit card on file.

Repeated Late Cancellations: To maintain a fair schedule, clients with three late cancellations/no-shows may be required to prepay for future services at the time of booking. This pre-payment is non-refundable for any violation of the cancellation policy.

Rescheduling: If you need to reschedule your appointment, please do so at least 48 hours in advance. Any pre-payment made will be credited towards your future service at Superior Self Aesthetics and Wellness.

 

As a courtesy to our providers, for non-group appointments, please provide a minimum of forty-eight (48) hours notice should you need to cancel or reschedule an appointment. You will be charged $100 if an appointment is canceled or rescheduled less than forty eight (48) hours in advance or if there is a no-show. For group appointments, there is a seventy two (72) hour cancellation policy, and the credit card on file will be charged $200 if canceled less than seventy two (72)  hours in advance. If you arrive more than ten (10) minutes late for your appointment, you may be required to reschedule to avoid disrupting other clients’ appointments. Please note, if you completed your Good Faith Examination Televisit, the cost of $100 is nonrefundable.

 

Why This Policy Matters:

We understand that scheduling changes can happen. However, this policy allows us to continue providing the highest quality service for you and all our patients. It also respects the valuable time of our team members who are often booked weeks in advance.

 

By scheduling an appointment, you are acknowledging and agreeing to our cancellation/no-show policy.

Pricing & Prepayments Policy:

Pricing for products and services is subject to change any time. Once prepaid, you have up to 1 year to redeem the purchased service(s).

If there are any payment issues with a client’s credit or debit card  – chargebacks, fraud issues, etc., we cannot accept credit or debit cards from the client and require payment in cash.

Refund Policy:

At Superior Self Aesthetics and Wellness, we strive to deliver exceptional results you’ll love. We understand that aesthetic goals can vary, and while we customize treatments to achieve your desired outcome, individual results may differ.

If you would like to return your purchased skincare product, return the item in its original packaging unopened within thirty (30) days of purchase. We do not accept returns on opened or used products. We do not accept returns or exchanges on gift cards.

Prepayments are refundable within ten (10) days of purchase. Any aesthetic/wellness services provided by Superior Self Aesthetics and Wellness are non-refundable, regardless of satisfaction or outcome.

Child Policy:

To ensure the safety of children and the enjoyment of all clients, we ask that parents or guardians make other arrangements for children while receiving their services. However, we do understand if there are circumstances where this may not be possible.

Pet Policy:

For the health and safety of our clients, we have a No-Pets policy. Only working service animals are permitted.

Content:

By posting or submitting content on or to Superior Self Aesthetics and Wellness’ website, you grant Superior Self Aesthetics and Wellness a non-exclusive, royalty-free, perpetual, irrevocable, and fully sublicensable right to use, reproduce, modify, adapt, publish, translate, create derivative works from, distribute, and display such content throughout the world in any media.

Consultations:

To ensure you’re completely satisfied with your experience, we offer consultations before each service. This allows us to discuss your expectations and tailor the treatment plan accordingly. Our consultations include education about your unique skin type, a comprehensive discussion about all options available to you, both immediate and long term, and a clear plan of action based on your individual goals. If you have any questions or concerns after your appointment, please don’t hesitate to contact us. We’re committed to providing excellent service and will work with you to address any questions.

Indemnification:

You agree to defend, indemnify and hold harmless the Company, its affiliates, subsidiaries, and each of its and their respective directors, officers, shareholders, managers, employees, agents, partners, representatives, licensors, successors, and assigns (collectively, “Related Parties”) from any claim, demand, loss, award of damages, expense, or cost (including reasonable attorneys’ fees) that arises out of:

(a)Your use of, inability to use or access, or reliance on, the Platform or the Services, or any goods or services obtained therefrom;

(b) Your provision or receipt of a Treatment;

(c) Your travel in connection with the provision or receipt of a Treatment;

(d) Your interaction with any other User;

(e) Any claim that you violated any provision, term, condition, covenant, warranty, or representation in this Agreement;

(f) Any violation of this Agreement by you or any other person using your User Account, whether or not such usage is expressly authorized by you;

(g) Any harm to person or property resulting from your acts or omissions, whether such acts are intentional, negligent, or otherwise;

(h) The Company’s use, disclosure, or preservation of User Content; or

(i) Your violation of any rights of another, including intellectual property rights.
